Submit a Public Records Request

This page is for general public records requests, if you are requesting police records please utilize the options here


Step 1.Go to the online Public Records Request form

Step 2.Click on the button titled "General"

This will open the online Public Records Request form

Step 3.Complete Public Records Request form

Please complete all areas of the form and include a detailed description of your request.

Please provide an email address for electronic notification.

Step 4.Click Submit

Your request will be routed and processed. You will be notified (via email if provided) when the request has been routed and again when the request is completed. 

Step 5.Fee Payment if Applicable

If there are any applicable fees associated to the request an invoice will be provided with the completion notice. All fees must be paid before records are released. 

Possible Fee Notice

No fee shall be charged for the inspection of public records. However, there is a charge for copies of records and Commercial purpose designation. Please refer to Administrative Directive #5(PDF, 1MB) for a list of all applicable fees.

Commercial purpose means the use of a public record for the purpose of sale or resale or for the purpose of producing a document containing all or part of the copy, printout or photograph for sale or the obtaining of names and addresses from public records for the purpose of solicitation or the sale of names and addresses to another for the purpose of solicitation or for any purpose in which the purchaser can reasonably anticipate the receipt of monetary gain from the direct or indirect use of the public record. Commercial purpose does not mean the use of a public record as evidence or as research for evidence in an action in any judicial or quasi-judicial body as defined by A.R.S. 39-121-03.D.

Step 6.Receiving the Records

Requested records will be sent in electronic format if that is their original form (*fees may apply, see Administrative Directive #5(PDF, 1MB)). If the original form is paper you will be notified when the documents are ready for review. There is no fee to view documents. Should copies be requested the applicable fees listed at the link above would apply.
